lang/org: add goto-visible & refile-to-visible commands

- Bind 'gsh' to goto-visible command in org-mode
- Bind '<localleader> g v' to goto-visible
- Bind '<localleader> r v' to refile-to-visible
- Add avy package to lang/org module
